Gain consent from presenter before adding page
From Programapedia
An author must have first hand experience of a program or presenter in order to add it to Programapedia. Authors are also encouraged to obtain a signed Vendor Release Form from the presenter before their personal information is posted on Programapedia (e.g. address, phone number, email address, etc.). In addition to getting consent, this form allows the author to gather important information from the presenter that will make the presenter and/or program articles more descriptive and in turn a more valuable tool for program sharing. Presenters and programs entered retrospectively do not need a Vendor Release Form.
- Vendor Release Form (use this version if presenter will be submitting form electronically)
- Vendor Release Form (use this version if presenter will be submitting a paper copy of the form)
